Objectives: The influence of social determinants on Health beyond the Healthcare sector is increasingly recognized worldwide. In Iran, the mandatory Health Impact Assessment (HIA) report, known as ‘Health annex’, evaluates the effects of development projects on Health primarily focusing on environmental factors and offering recommendations to mitigate these effects. However, Health is influenced more significantly by other factors than by environmental ones. This policy brief proposes the expansion of HIA practices to encompass programs, policies, and laws, emphasizing a Health-oriented policymaking approach that addresses all Health determinants.Methods: This policy brief is based on prior research to design a Health Impact Assessment model for development projects in Iran, involving interviews with 14 experts and the participation of 102 academics. This model has been internationally validated through the participation of 38 international experts.Results: Drawing on national and international evidence, the policy brief outlines phased implementation, stakeholder engagement strategies, and monitoring mechanisms to effectively integrate Health considerations into all policies.Conclusion: A Health-oriented policymaking approach that considers all determinants of Health is proposed. The proposed approach aims to enhance multisector collaboration, improve public Health equity, and support sustainable decision-making.
